Updating Parishioner's Personal Information Forms

In reviewing our files during our recent down time, it has been noted our parish record form was missing some key legalities in relation to personal privacy concerns.

In September, we will be asking each parishioner to complete and sign a new Personal Information Form. The forms will be sent out by email (or mailed for those who do not have an email) and the form can be completed online or printed off, signed and returned to the church office.  In the past, one form was completed per family but now it is necessary for all parishioners to complete an individual form. Dependent children are to be included with the parents/guardians.
